Overview
Forms on your Psyke pages let you capture lead information β names, emails, phone numbers, and more β directly from visitors. This article covers how to add a form, customise its fields, connect it to your CRM, and ensure GDPR compliance.
Adding a Form to a Page
Open a page in the Psyke editor.
Click the + button to add a new section or element.
Select Form from the element library.
The form will appear on the page with default fields (name and email).
Customising Form Fields
In the form editor panel:
Click Add Field to add new inputs (text, email, phone, dropdown, checkbox, etc.).
Click any field to edit its label, placeholder text, and whether it is required.
Drag fields to reorder them.
Click the trash icon to remove a field.
Setting the Submit Button
Click the submit button to edit its label (e.g. Get a Free Quote, Download Now) and its colour. You can also set a redirect URL or a thank-you message that appears after submission.
Adding Validation
Mark fields as Required to prevent blank submissions. For email fields, Psyke automatically checks that the entry is a valid email format.

GDPR Consent Checkbox
If you operate in the EU or UK, you must obtain explicit consent before storing or using personal data. To add a consent checkbox:
Add a Checkbox field to your form.
Label it clearly, e.g. I agree to receive communications from [Your Company] in accordance with our Privacy Policy.
Link the words Privacy Policy to your actual privacy policy page.
Mark the checkbox as required.
Viewing Form Submissions
All submissions are saved in your Psyke dashboard. Go to your campaign, click on the page, and select Leads or Submissions to view and export responses.
